A 20 Year Old Denture, A Changed Face, And A Remarkable Transformation At Swissedent Denture Clinic

At Swissedent Denture Clinic, we often meet patients who have quietly adapted to dentures that no longer serve them. They chew a little less confidently, smile a little less freely, and over time, their face subtly changes shape. This case is a perfect example of why dentures should never be seen as something you fit once and forget about for decades.

This story is about a gentleman who had worn the same set of dentures for around 20 years. Twenty years is a long time for any dental appliance. Teeth wear down, acrylic thins, and most importantly, the jawbone and soft tissues underneath change. By the time we became involved, those dentures were not just worn out, they were actively altering his facial appearance and comfort.

What Happens When Dentures Are Worn For Too Long
Dentures are designed to replace teeth, but they also play a vital role in supporting the lips, cheeks, and overall facial structure. When dentures are worn for many years without being reviewed or replaced, several things happen.

The biting surfaces wear flat, reducing chewing efficiency. The vertical height of the bite collapses, meaning the distance between the nose and chin shortens. This can give the face a sunken, tired appearance. The lips lose support, the cheeks fall inward, and lines around the mouth deepen. Many patients think this is just part of getting older, but very often it is denture related.

In this case, the existing dentures had lost most of their shape and structure. The acrylic teeth were heavily worn, the base was thin, and there was very little soft tissue support left. The patient had adapted, as many people do, but adaptation does not mean optimal function or appearance.

Being Asked To Step In Clinically
This patient was being treated at another dental practice, and I was asked to come in specifically for the key clinical stages. These included the bite registration, the secondary impression, and the try-in stage. These steps are absolutely critical in denture success, especially in complex or long-standing cases.

Taking over at these stages allows us to assess not just the mouth, but the face, jaw relationship, muscle balance, and how everything works together. Dentures are not just about teeth sitting on gums. They are about harmony between function, aesthetics, and facial support.

Rebuilding The Bite And Facial Support
One of the first things we addressed was the bite. After 20 years of wear, the bite height had collapsed significantly. Restoring this correctly is essential. Too little height and the face remains unsupported. Too much and the patient becomes uncomfortable, strained, or unable to adapt.

This is where experience matters. As a Clinical Dental Technician, my role bridges the gap between clinical assessment and technical design. We carefully re-established the correct jaw relationship, ensuring it was comfortable, stable, and natural for the patient.

Why Secondary Impressions Matter
Secondary impressions allow us to capture the soft tissues in function. This means the denture fits the muscles, not fights them. Poor impressions lead to loose dentures, sore spots, and lack of confidence when speaking or eating.

In this case, precise secondary impressions helped us design a denture base that hugged the anatomy correctly, improving retention and stability. This is one of the reasons the final denture fitted so well and felt secure from the start.

The Importance Of The Try In Stage
The try in stage is where everything comes together. It is the opportunity to assess aesthetics, phonetics, bite, and facial support before the denture is finalised.

At the try in, we could clearly see how restoring the correct height and support began to lift the face. The lips were fuller, the cheeks were supported, and the overall expression looked healthier and more relaxed. This is often the moment patients and their families truly see the difference.

Designing A Denture That Looks Natural
One of the biggest compliments we hear at Swissedent Denture Clinic is when someone says, it does not look like dentures. That is never an accident.

For this patient, we deliberately designed the denture to be more bolbus. This means the shape was fuller and provided proper support to the soft tissues. Many older dentures are made too thin in an attempt to keep them small, but thin dentures often fail to support the face.

We also utilised the patient’s natural facial features and jaw relationship. Instead of forcing a generic smile, we worked with what nature had already given him. This included copying small details such as natural spacing between certain teeth, known as diastemas. These tiny gaps make a huge difference to realism. Perfectly straight, perfectly even teeth often look artificial, especially in older patients.

By respecting these details, the final result looked like his smile, just refreshed and supported.

The Moment That Really Mattered
One of the most rewarding moments came not just from the patient, but from his wife. She was genuinely delighted with the result. In her words, the denture looked natural, fitted beautifully, and lifted his face up. That reaction tells us we got it right.

Dentures affect not only how patients feel about themselves, but how they are seen by the people closest to them. When a partner notices a positive change, it reinforces confidence and satisfaction in a way no technical measurement ever could.
